Permission to leave unattended (v1.1)

    You have indicated on your application form that you wish for your child to leave the Silverbox After School Club / Holiday School without being accompanied by a parent/carer/adult.

    It is our recommendation that all children are collected by a responsible adult, especially during the darker winter months.

    We do understand however that everybody’s situation is different and sometimes students may be required to leave without supervision.

    If this is the case, then we require the following form to be completed in order for us to allow your son/daughter to leave at the end of our workshop time without an adult.
    1. I give permission for my son/daughter to leave the Silverbox After School Club / Holiday School without a parent/carer/adult from the school/venue.
    2. ​​I understand that Silverbox Film Ltd is in no way responsible for personal loss or injury when my child has left the premises.
