Permission to leave unattended (v1.1)

You have indicated on your application form that you wish for your child to leave the Silverbox Multimedia without being accompanied by a parent/carer/adult.

It is our recommendation that all children are collected by a responsible adult, especially during the darker winter months.

We do understand however that everybody’s situation is different and sometimes students may be required to leave without supervision.

If this is the case, then we require the following form to be completed in order for us to allow your son/daughter to leave at the end of our workshop time without an adult.

If we have not received this form we will not be able to let your son/daughter leave alone so it is of great importance that we receive this.
